The R on T Phenomenon

Commotio cordis, a rare, life-threatening disruption of heart rhythm caused by a sudden, blunt impact to the chest directly over the heart

The R-on-T phenomenon is a specific pattern seen on an electrocardiogram (ECG) that happens when a premature ventricular contraction (PVC) occurs during the relative refractory period, which is the last part of the T wave from the previous heartbeat. This timing is very important because the heart is still recovering and can be more prone to electrical disturbances. If a PVC occurs at this critical time, it might trigger more serious heart rhythm problems, such as ventricular tachycardia or even ventricular fibrillation. For the health care provider, it’s important to understand this phenomenon because recognizing the R-on-T pattern can help evaluate the risk of severe cardiac events. Close monitoring of patients with a history of heart issues or those taking specific medications can help prevent related risks, highlighting the significance of identifying this ECG pattern in nursing practice.

The T wave indicates when the heart’s ventricles are getting ready to reset their electrical charge. If a premature beat (PVC) occurs during the downward slope or peak of this T wave, some parts of the heart muscle may have fully recovered while others are still partially affected. This uneven recovery can lead to a conduction block and initiate a dangerous loop of electrical signals, known as reentry. This R-on-T phenomenon can result in rapid, chaotic heart rhythms like polymorphic ventricular tachycardia (VT), ventricular fibrillation (VF), or Torsades de Pointes, especially if the QT interval is longer than normal.

Rest in Peace to the talent and our deepest condolences to the family.
